Order Forms & Payment

• Download and complete our New Project Order Forms [.pdf format]. 
  · The Order Forms require signatures, so you will need to print them and then fill them in.
  · Important information about paying for your order.
  · Additional information about how to fill out the Intellectual Property Rights (IPR) form.
   
• You need to send your order forms to us - and we always recommend that you send us your quotation as well. 
  Fax the paperwork to us at 604.261.3313 or you can scan/e-mail them to us at orders(at)CDman.com.
   
• Do you have a *deadline* when you absolutely must have your order? 
  If "yes" then we need to know all the details as soon as possible. Learn more about our standard turn times.
